Revity Golf Event Giveaway — Official Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
1. Sponsor: Revity Marketing Agency ("Sponsor"), [SPONSOR ADDRESS], American Fork, UT.
2. Eligibility: Open to legal residents of the United States who are 18 years of age or older at the time of entry. Employees of Sponsor and their immediate family members are not eligible. Void where prohibited.
3. Entry Period: Entries may be submitted at the Sponsor's booth at [EVENT NAME] on [EVENT DATE] ("Entry Period"). One entry per person.
4. How to Enter: Visit the Sponsor's booth and complete the entry form with your name, email, phone number, and website URL. No purchase, payment, or golf participation is required to enter.
a. Grand Prize (1 winner): A website build valued at $6,500, or any Revity Marketing service of equal value, redeemable within 12 months of the drawing. Approximate Retail Value ("ARV"): $6,500.
b. Second Prize (1 winner): One (1) golf bag. Winner will be selected by live raffle drawing at the event luncheon. ARV: [GOLF BAG VALUE].
c. Third Prize: Every entrant who submits a valid, complete entry via the online form during the Entry Period will receive a complimentary marketing audit, competitor analysis, and strategy session from Revity Marketing. ARV: $549 each.
6. Winner Selection: The Grand Prize winner will be selected at random from all valid entries received during the Entry Period, within seven (7) days following the event. The Second Prize winner will be drawn live at the event luncheon. Odds of winning the Grand Prize depend on the total number of eligible entries received.
7. Winner Notification: The Grand Prize winner will be notified by phone and/or email using the contact information provided at entry. In accordance with Utah's Prize Notices Regulation Act, winners will receive written notice disclosing the prize's value and, where applicable, the odds of winning, before any additional information is requested. The winner must respond within thirty (30) days of notification to claim the prize, or an alternate winner may be selected.
8. Tax Obligations: Winner is solely responsible for all applicable federal, state, and local taxes on any prize received. Sponsor will issue an IRS Form 1099 for any prize with a fair market value exceeding $600, as required by law.
9. General Conditions: Sponsor reserves the right to verify eligibility, disqualify any entry it believes to be fraudulent, and cancel, suspend, or modify this promotion if it is unable to run as planned. By entering, participants agree to be bound by these Official Rules and agree to be contacted by Sponsor regarding their entry and results.
10. Privacy: Information collected in connection with this promotion is subject to Sponsor's Privacy Policy.
